GAD-7 (General Anxiety Disorder-7)

The Generalized Anxiety Disorder 7 (GAD-7) is a self-reported questionnaire for screening and severity measuring of generalized anxiety disorder (GAD).

The GAD-7 questionnaire was introduced in OpenEMR 6.0.0 Patch 2. It is an API-based form.

Enable the form

Go to Administration / Forms / Forms Administration and scroll down to Unregistered / GAD-7. Select the following:

1. Register - this changes it to "enabled"
2. Install DB - this changes it to "disabled"
3. Disabled - this changes it to enabled

Use the questionnaire

The GAD-7 questionnaire is available from the Clinical menu within an encounter, or from the Patinet/Client / Visit Forms / GAD-7 menu when a patient/client has been selected. The completed questionnaire is saved within the currently open encounter.
